Intrauterine insemination (IUI) is one of the methods of Assisted Reproductive Technology (ART)- a form of artificial insemination wherein the washed sperm from the male partner is inserted into the womb of the female partner when she is ovulating (at the time of the release of one or more eggs from ovaries) using a catheter. This allows the eggs to get fertilized with the sperm and develop into a healthy embryo, eventually leading to pregnancy. The procedure requires coordination with the woman to keep a check on her ovulation time and use of fertility medications.
IUI treatment is usually recommended in case of unexplained infertility along with intake of medications that induce ovulation when other fertility treatments are not successful.
Subfertility of males: Mild male factor infertility is when it has been analyzed from semen sample that the male partner is having below-average sperm count, weak sperm motility, or abnormalities in sperm size and shape. In such cases, Intrauterine insemination can overcome the problems by using donor sperms that are kept frozen in the specialized fertility centers and thawed before use; or use sperm retrieval methods that separate the healthy and highly-motile sperms from low quality.
Endometriosis associated with infertility: Some medications and IUI treatment can work together in patients having endometriosis (a condition in which the tissues lining the uterine cavity grow outside) to obtain good quality eggs. Cervical related infertility: The cervix is the opening between the vagina and uterus which is responsible for creating an ideal environment for assisting sperm motility to allow sperms to reach fallopian tubes from the vagina. Infertility can occur when the female has hostile cervical mucus (that is thick mucus secretion) which impedes the movement of sperms. Thus IUI treatment can bypass the cervix, thereby increasing the number of sperms available for fertilization with egg.
Ovulation problems: IUI is beneficial for women who want to get pregnant but are facing problems with ovulation i.e. no or little release of eggs.
Semen allergy: It is a rare case that the female is allergic to proteins present in the semen of her partner. IUI treatment can help to some extent, as most proteins are removed in semen samples before the insertion of sperm.
Pre-treatment: The semen sample is prepared by either using the thawed frozen donor sample or the ejaculated sample provided by the husband. Healthy and concentrated sperms are selected for the IUI procedure which increases the likelihood of successful pregnancy. On the other side, the female partner is monitored for the signs of ovulation. Blood and urine tests may be performed for checking the hormone levels and infections. Ultrasound can also be used to visualize ovaries and the growth of eggs. IUI is performed only after detecting ovulation i.e. within two days of egg release.
During treatment: The woman is asked to lie on the examination table with legs apart stirrups) and the speculum is inserted into her vagina. Next, a vial containing the healthy sperm is attached to one end of the catheter (a thin, long, flexible tube). The catheter is inserted into the vagina into the uterus through the cervical opening and the semen is pushed inside the uterus. After successful insemination, the catheter and speculum are removed.
The entire procedure takes about 15 to 20 minutes with no need for medications or pain relievers.
The inseminated woman shall rest on her back for a brief time after the completion of the procedure. After the procedure is over, the individual can return to her daily routine. The positive results of pregnancy can be tested after two weeks of performing this procedure.
