Microfluidic sperm selection, also known as microfluidic sperm sorting, is an advanced sperm preparation technique used during fertility treatments such as IVF and ICSI. The semen sample is passed through a specialised device containing microscopic channels that help separate highly motile sperm with favourable characteristics from the rest of the sample.
When couples ask what microfluidic sperm sorting is, it is important to understand that the technique is used to improve sperm selection before fertilisation. It does not replace IVF or ICSI and is not required for every fertility treatment cycle.

Microfluidic Sperm Sorting Process Step by Step
The microfluidic sperm sorting process takes place inside the embryology laboratory. The exact approach may vary depending on sperm concentration, motility, morphology, DNA fragmentation, previous treatment history, and the type of fertility treatment planned.
Step 1. Semen Sample Collection & Assessment
A semen sample is collected and evaluated in the laboratory. Embryologists assess sperm concentration, motility, morphology, and other relevant parameters before deciding how the sample should be prepared.
Step 2. Sample Placement in the Microfluidic Device
The semen sample is placed into a specialised microfluidic sperm sorting device containing microscopic channels designed to allow motile sperm to move through them.
Step 3. Natural Sperm Movement Through Microchannels
Sperm with good progressive motility move through the microchannels, helping separate a population of motile sperm from sperm with poorer movement and other less favourable characteristics.
Step 4. Collection & Assessment of Selected Sperm
The sperm that successfully pass through the channels are collected and assessed by the embryology team. The selected sperm population may show better motility and lower sperm DNA fragmentation than the original sample.
Step 5. Use During IVF or ICSI
Depending on the treatment plan, the selected sperm may be used for fertilisation during IVF or further selected for injection directly into a mature egg during ICSI.
What Couples Should Know
Microfluidic sperm selection can be useful in selected fertility cases, particularly where sperm quality may be affecting fertilisation or embryo development. However, couples should understand what the technique can and cannot influence before it is included in treatment.
1. Microfluidic Sperm Sorting Is Not Needed for Every Couple
The technique may be considered when there is increased sperm DNA fragmentation, reduced sperm motility, abnormal sperm morphology, male-factor infertility, previous fertilisation problems, poor embryo development, or an unsuccessful IVF or ICSI cycle.
2. It Helps Select Sperm but Does Not Treat the Underlying Cause
Microfluidic sperm selection helps the embryology team obtain a selected population of sperm with favourable characteristics. It does not directly treat the underlying reason for poor sperm quality or male infertility.
3. Can Microfluidic Sperm Sorting Reduce DNA Fragmentation?
Microfluidic sperm sorting may help select sperm with lower DNA fragmentation compared with the original semen sample or some conventional sperm preparation techniques. However, the degree of improvement can vary between patients.

Is microfluidic sperm selection better than ICSI?
Microfluidic sperm selection and ICSI are not competing fertility treatments. Microfluidic sperm selection is a laboratory method used to separate highly motile, better-quality sperm from a semen sample. ICSI is a fertilisation technique in which an embryologist injects one selected sperm directly into a mature egg.
Microfluidic sperm selection may therefore be performed before ICSI to help prepare the sperm sample. Studies suggest that it can improve selected sperm parameters and reduce sperm DNA fragmentation, but current evidence does not prove that it improves pregnancy or live-birth rates for every patient.
What is the success rate of microfluidic sperm selection?
There is no fixed success rate for microfluidic sperm selection because it is an additional sperm-preparation method rather than a complete fertility treatment. The chances of successful fertilisation, pregnancy and live birth depend on several factors, including the woman’s age, egg quality, sperm quality, embryo development, uterine health and previous fertility history.
Research indicates that microfluidic techniques may help select sperm with better motility and lower DNA fragmentation. However, more high-quality evidence is required to confirm whether these laboratory improvements consistently result in higher pregnancy and live-birth rates.
Will all of our eggs be injected after microfluidic sperm selection?
When microfluidic sperm selection is used as part of an ICSI cycle, our embryology team makes every appropriate effort to inject the available mature eggs. However, only eggs that have reached the correct stage of maturity can usually be injected with sperm during ICSI.
The number of mature eggs may be lower than the total number retrieved during the egg-collection procedure. Immature or unsuitable eggs may not be injected because they have a reduced likelihood of successful fertilisation. Your fertility specialist and embryologist will explain the number of eggs retrieved, the number that are mature and the number successfully injected.
Who may consider microfluidic sperm selection?
Microfluidic sperm selection may be considered for couples experiencing male-factor infertility, elevated sperm DNA fragmentation, repeated miscarriage, poor embryo development or unsuccessful previous IVF or ICSI cycles. It may also be discussed when conventional sperm-preparation techniques have not produced the desired sperm quality.
However, the technique is not necessary or suitable for every patient. In samples with very low sperm numbers or poor motility, another preparation method may be more appropriate.
How is microfluidic sperm selection performed?
Sperm preparation is a laboratory process used to separate suitable motile sperm from seminal fluid, immotile sperm, debris and other unwanted components of the semen sample. Conventional preparation methods may include density-gradient centrifugation or the swim-up technique.
Microfluidic sperm selection is a different preparation method. The sample passes through microscopic channels without repeated centrifugation, allowing motile sperm to separate according to their natural movement. This minimal-handling approach may help reduce oxidative stress and select sperm with lower DNA fragmentation. The collected sperm are then evaluated and used for IVF or ICSI according to the treatment plan recommended by the fertility specialist.
What does sperm preparation or “sperm washing” mean?
A prepared semen sample is placed into a specially designed microfluidic device containing microscopic channels. Motile sperm move through these channels, which are designed to imitate certain aspects of the natural sperm-selection process within the female reproductive tract.
Sperm that successfully travel through the channels are collected from a separate chamber and assessed by the embryology team. The selected sperm may then be used for conventional IVF or individually injected into mature eggs during ICSI. The procedure is performed entirely in the laboratory and does not involve any additional invasive procedure for the patient.
