What Is Preimplantation Genetic Testing (PGT)?
Preimplantation genetic testing (PGT) is a form of embryo genetic testing performed on embryos created through IVF before embryo transfer. Depending on the reason for testing, PGT can assess embryos for chromosome-number abnormalities, specific inherited genetic conditions, or structural chromosome rearrangements.
Because embryos need to be created and tested in the laboratory, IVF and preimplantation genetic testing are performed together. PGT provides additional genetic information that may help guide embryo-transfer decisions, but it does not test for every possible condition or guarantee pregnancy.
Types of PGT Genetic Testing: PGT-A, PGT-M & PGT-SR
There is no single PGT test that answers every genetic question. The appropriate form of preimplantation genetic testing PGT depends on what doctors are specifically looking for.
- PGT-A: Chromosome Number Testing
PGT-A, or preimplantation genetic testing for aneuploidy, evaluates whether an embryo has the expected number of chromosomes. It may be discussed in selected IVF cases but is not routinely recommended for every patient. - PGT-M: Testing for a Known Genetic Condition
PGT-M may be considered when one or both partners have a known risk of passing on a specific single-gene disorder. Testing is designed around the particular genetic condition identified. - PGT-SR: Testing Structural Chromosome Changes
PGT-SR may be used when one partner carries a structural chromosome rearrangement, such as certain translocations or inversions, that could affect the chromosome balance of an embryo.

IVF and Preimplantation Genetic Testing: Process Step by Step
The preimplantation genetic testing process takes place within an IVF cycle. Fertility specialists, embryologists and genetic-testing teams work together from embryo creation through testing and future embryo transfer.
Step 1. Fertility & Genetic Evaluation
The couple's medical history, reproductive history, family history and reason for considering PGT are reviewed. Genetic counselling or additional testing may be recommended before beginning treatment.
Step 2. IVF, Egg Retrieval & Fertilisation
The ovaries are stimulated to develop multiple eggs. The eggs are then retrieved and fertilised through IVF or ICSI so embryos can be created for further assessment.
Step 3. Embryos Are Cultured to the Blastocyst Stage
After fertilisation, embryos are monitored in the laboratory for several days. Suitable embryos may continue developing until approximately Day 5 or Day 6, when they reach the blastocyst stage.
Step 4. Embryo Biopsy Is Performed
A small number of cells are carefully collected from the outer part of a suitable blastocyst. These cells are then sent for the required embryo genetic testing.
Step 5. Genetic Analysis Is Carried Out
The biopsied cells are analysed according to the required test—PGT-A, PGT-M or PGT-SR. Embryos are generally frozen while the genetic testing results are awaited.
Step 6. Embryo Transfer Is Planned
After the results are available, the fertility team discusses which embryos may be appropriate for transfer. A suitable frozen embryo may then be transferred in a subsequent cycle.
What Couples Should Know
PGT can provide valuable genetic information in appropriately selected cases, but couples should understand both its potential role and its limitations before beginning IVF with PGT.
1. PGT Is Not Necessary for Every IVF Cycle
The reason for testing matters. PGT-M and PGT-SR address specific genetic risks, while routine PGT-A has not been shown to improve overall IVF outcomes for every patient.
2. PGT Cannot Detect Every Genetic Condition
The result relates to what the specific test is designed to assess. A reassuring PGT result does not mean that every possible genetic, chromosomal or developmental condition has been excluded.
3. Not Every Embryo Will Be Available for Testing or Transfer
Not every fertilised egg develops into a blastocyst, and not every blastocyst will necessarily provide a result suitable for transfer planning. The number of available embryos varies between cycles.
4. Some PGT Results May Need Further Interpretation
